WHAT IS REPORTABLE "ABUSE": Physical, sexual, or mental abuse or neglect by a parent or a "person responsible for a child's welfare" (includes the child's guardian, foster parent, an adult who resides in the same home as the child, a person providing care in a day-care or residential facility).
